October 4-6, 2007

Selected Presentations

The American Society of ExtraCorporeal Technology  in conjunction with the International Consortium for Evidence-Based Perfusion held the annual Best Practice Perfusion 2007 at the Delta Montreal, in Montreal, Quebec, Canada on October 4-6, 2007. The following are a few of the presentations that were given during this meeting.

Session 1: Quality Improvement Training Skills

Platelet Preservation: Do Perfusion Strategies Really Make a Difference?
Gordon R. DeFoe, CCP

Objective: To share the status of the guideline writing group’s progress on the topic of platelet preservation. Proposed recommendations will be discussed in an interactive manner with the audience.

Session 2: Quality Improvement Training Skills

Using Evidence and Context Knowledge to Change the Culture of your Microsystem
Robert C. Groom, MS, CCP

Objective: To explore the essential role of evidence and context knowledge as the foundation for change.

Session 3: Cycles of Change in Your System of Delivery Care

The Clinical Microsystem - Where Quality is Made or Lost
Robert C. Groom, MS, CCP

Objective: To explain the basic concept of a clinical microsystem and show how an instrument panel may be used to help the multidisciplinary team improve.

Session 4: Statistical Research Control Theory

Using Statistical Run Charts to Improve Clinical Practices
Robert A. Baker, PhD, CCP(Aus)

Objective: The presentation is designed to demonstrate the benefits of the use of statistical run charts in everyday clinical practice. The incorporation of run charts into a quality improvement process for monitoring of clinical practice will be presented.

Session 6: Oral Abstracts

From Theory to Clinical Practice: A Change in Myocardial Preservation Strategies Using the Quest Medical MPS 2 System
Jim McDonald

Objective: To provide a framework for interactive discussion with participants as to how we progressed from the philosophy and theory of change to our final clinical initiation of change in our myocardial preservation srategies.

The Clinical Relevance of Ventilation During CPB in the Prevention of Lung Dysfunction in the Post-operative Period
Julie Gagnon, BSc, MSc

Objective: The aim of this study is to determine if ventilation during CPB prevents the selective endothelium mediated lung dysfunction in humans, and to determine the clinical relevance of ventilation per CPB.

Session 7: Adult & Pediatric Clinical Registries for Cardiac Surgery

A Regional Perfusion Registry – The Good and the Bad
Gordon R. DeFoe, CCP

Objective: Provide overview of our ten years of experience with a regional (tri-state) perfusion registry. We will review some areas in which we have made great strides (nadir hematocrit on bypass, blood usage) and others where much remains to be done (emboli reduction, myocardial preservation). Attendees will learn some of the pitfalls of registry design and data collection, and how to “generate light, not heat.”

International Perfusion Registry – Opportunities and Potential Pitfalls
Donald S. Likosky, PhD

Objective: Provide an overview of a proposed international perfusion registry under the ICEBP umbrella. Attendees will be oriented to the pros and cons of this initiative. A proposed registry data form and experience using it will be shared. Initiative to roll out this registry to a larger group of centers will be put forth.

Session 8: Transparency and Public Reporting

History of Transparency and Public Reporting and Terminology 101
Veena Guru, MD

Objective: Provide overview and history, both in North America and abroad regarding the transparency movement. Included in this will be where different countries are in the evolution of this movement. Provide definitions for key terms, such as accountability, stakeholders, etc. Also include outside influences, such as IHI, NQF, etc

Defining the Metrics
Gordon R. DeFoe, CCP

Objective: Discuss the role of perfusion in the context of transparency. Provide overview of other metrics in cardiac surgery, and evidence-base supporting them. Discuss what makes a good metric. Describe what metrics would be helpful to key stakeholders, and pathways that we might take as a profession and set of professionals in developing these metrics.

Critical Appraisal of Risk-adjusted Analysis and Public Reporting of Outcomes
in Cardiac Surgery

Jean-Yves Dupuis, MD, FRCPC

Objective: Through an overview of basic epidemiologic principles, this lecture addresses the uncertainty associated with outcome predictions. It highlights the limitations of using large databases, complex statistics and public reports to compare outcomes between institutions and improve quality of care.

Stakeholder Views and Roundtable Discussion
Donald S. Likosky, PhD

a. Surgeon
b. Senior Perfusionist
c. Epidemiologist
Objective: Have key stakeholders provide their perspective on public reporting, including how valuable it is, how might metrics be used, what metrics would be helpful, what are the ramifications of public reporting, is this marketing or quality assurance, what obligation do we have as a profession to support public reporting.
